Purpose

To ensure the qualifications of sampling and testing personnel and to evaluate the performance of the qualified sampling and testing personnel and testing equipment at the Quality Control and Verification testing level. The program covers sampling procedures, testing procedures, and testing equipment.

 

General Information

Technicians who perform sampling and testing for the Authority on construction projects must be qualified by the Construction Training Qualification Program (CTQP).  Qualifications can be verified by checking their web site at www.ctqpflorida.com. Acceptance Program sampling and testing includes Quality Control (QC), Verification (V), and Resolution (R) testing.

 

Periodic IA evaluations are intended to assure continuance of qualifications, used in acceptance of the materials.  CTQP qualifications are subject to suspension according to the Construction Training and Qualification Manual (CTQM).

 

Performing Evaluations

Testing Equipment:

The testing equipment shall be evaluated by using one or more of the following: Calibration Checks, Split Samples, or Proficiency Samples.

 

Testing Personnel:

Testing personnel shall be evaluated by Proficiency Samples, or Observations and Split Samples.  When Observations & Split Samples are used as a means of evaluation, an evaluation is considered complete when any one of the two evaluation methods are used. The IA personnel shall maintain a record of all performance evaluations performed.

 

The Field IA Checklist will be used to document the results of the evaluation and will be completed within five working days. The evaluation will be by proficiency samples, observation, or split samples. The evaluation by observation will be considered satisfactory if all items on the test procedure checklist(s) are completed correctly. The evaluation by split sample will be considered satisfactory if the test results meet the comparison criteria as defined on the checklist(s). The evaluation shall be considered satisfactory if the test results are within two standard deviations of the grand average value for all proficiency samples in that evaluation.


Deficient Evaluations

Sampling and testing technicians, who fail an evaluation, will be scheduled for re- evaluation. The re-scheduled evaluation can be in a different test method under the same qualification area. A failing evaluation by proficiency sample will not be followed by another proficiency sample for re-evaluation.  In order to maintain qualification status, technicians will be evaluated by another member of the IA team. Qualified technicians who demonstrate erroneous field sampling, testing or reporting techniques shall be subject to review by a qualified, Independent Assurance (IA) person. If the qualified, IA person files a failing test report, stating that he/she has witnessed a qualified technician perform sampling, testing or reporting in an erroneous manner, then the qualified technician shall be subject to the following per individual qualification:

 

1st Error- Verbal (documented) warning from the witnessing IA personnel.  IA personnel shall clearly explain the error to the technician and ensure that the technician understands and can perform the procedure properly.  If a subsequent observation is documented error free or no other errors are witnessed by a qualified IA person within one year (365 days), then the record of the 1st error shall be deleted from the technician's qualification record.

 

2nd Error- Written warning from the witnessing IA personnel. IA personnel shall clearly explain the error to the technician and ensure that the technician understands and can perform the procedure properly.  If a subsequent observation is documented error free or no other errors are witnessed by a qualified IA person within one year (365 days) then all errors for the individual qualification shall be deleted.

 

3rd Error- Suspension of qualification by the Authority.  If a subsequent observation is documented error free or no other errors are witnessed by a qualified IA person within one year (365 days) then the record of all errors shall be deleted. Should the 3rd error be determined valid, the Authority will suspend the technician's qualification.

 

Prior to having the qualification reinstated, the technician must attend and complete any applicable qualification course(s), any applicable written examination and any applicable proficiency examination. This includes meeting all initial, conditional requirements, as well as passing the written examination and proficiency examination.  One copy of the form shall be given to the employee, one copy to the employee's supervisor (or contractor's representative), and one copy retained in the Project records. Test results from a suspended technician will not be allowed for acceptance decisions.


Disqualification

Active, qualified technicians must conform to the Authority’s practices with regard to Quality Control operations. Should any qualified technician falsify records, his/her qualification will be subject to possible revocation by the Authority. Falsification of project related documentation may be subject to further investigation and penalty under State Statutes and Federal Law.

Notification that a qualification has been revoked shall be by "return receipt requested" mail. A copy of the notification shall be sent to the employee's supervisor (or contractor's representative. Anyone having their qualification revoked shall be eligible to apply for new qualification after a period of two years.

 

Comparison Testing:

Acceptable Comparison Tolerances are found in the specifications.

 

Proficiency Samples:

Acceptable tolerance for the test results is two standard deviations of the grand average value for all proficiency samples in that evaluation.
